All of our patient forms are available for you to download and fill out in the comfort of your home.
You can drop them off at our office when you’re in town, or just bring them with you to your first appointment.
Your very first visit with us will consist of a comprehensive examination by Dr. Callejo and the Artisan Dental Group team.
During this exam, we will check your oral anatomy, screen for gum disease, conduct an oral cancer screening, and take any necessary digital x-rays or intraoral images.
We’ve got everything you need to feel comfortable while you’re here! In addition to offering sedation options, we have other items like neck pillows and warm blankets to keep you comfortable while you’re resting back in the treatment chair.
Or, help yourself to a fresh-brewed cup of coffee or bottled water while you wait. Each treatment room is enhanced with the latest technology and education software to keep you up to date on all of your oral health care needs.
We’re happy to file insurance claims on your behalf. Our goal is to make all of our services affordable so that you don’t have to put off the dental care that you really need.
In addition to offering a cash discount, we also accept Visa, MasterCard, and Discover.
Patients also have the opportunity to finance their treatments through CareCredit and other third-party financing companies that provide short-term, no-interest credit for dental and medical care procedures.
